FrontlineSMS does this sort of thing. I have used it with Ushahidi for the same thing you are talking about. Perhaps you can look at the code and see what you need to modify.

You could also explain what you are trying to implement for a broader understand before we can tell if we know other apps that can fulfil the same.

FrontlineSMS is available for both Windows and Linux.
