In addition to what Brainiac suggests please go here to check your cname records: http://www.google.com/support/a/bin/answer.py?hl=en&answer=116393

I have a client where the cname records are set up correctly but they can not access their custom url from their office. After going back and forth with them on the phone I finally went over and discovered that indeed you could not reach the custom url or even their website from their office.
AfricaOnline DNS had not yet picked up the new servers more than a week later. A quick check by switching to OpenDNS confirmed this.

So check the cname records just in case
