One of agile principle is to welcome changing requirements, even late in development. You're handling a fixed cost project, and customer keep introducing more changes, what next?

I tend to think Agile is only appropriate for in-house development.

Based on your experience, which approaches do you use in your project management? Traditional waterfall or Agile?  Which one is effective in managing fixed price project?
