Had that exact problem with a 2011 macbook, exact to the detail you describe. Mine would suddenly surface, i restore and then the problem resurfaces. Not sure if it was ever sorted, but i suspect it had to do with hardware.

A hardware test shows that nothing is wrong (though i know that this is a very basic test), and I am currently trying to do a restore from Time Machine backup (6 hours away).
