I happen to have contributed to making a voucher application. I remember that it had a transactions table where among other things, it listed an customer details, IP address, who served the customer, what time, what they bought etc etc and most importantly their balance(with a suggestion of what they can redeem for).
So the TUSKYS ICT stuff should recheck their voucher application, and offer to pay the customer for their mistakes, because clearly this is a mistake that should NOT happen.
