Hi,

I had the same problem, but with custom ROMs at version 4.3 and 4.4. While the stk app is on the phone, I got it working (quite interestingly) by enabling the SIM PIN request on boot. Weird enough, its worked as a perfect solution to the stk issue on my various ROMs. It may hopefully work in your case too.

Call Safcom, usually mpesa/safaricom menus are an app sent to your phone :) - i stand to be corrected [].
I updated my MENU using same method and mine didnt vanish. Maybe share phone make.
