You have said it man!I do not think anyone EVER plans for failure!You do everything the way it should be done then hope it pans out!If it doesn't,you go back to the drawing board and make a few changes!
People, let us stop complaining. This is a common problem that happens during upgrade and it, IMHO, was not planned for. Let's give the firm where it's due and only complain when it's so necessary.
