The big boys in self publishing are probably createspace and ingramspark. Createspace is the easiest way to get on amazon kindle and physical books. Ingramspark has a wider online network(kindle, kobo, ibooks e.t.c) and bigger international footprint so you should look into using a combination of both to reach the widest market. 

I haven't done this but I love reading books and looked into it just this year. Turns out publishing is the easy part, writing and marketing is way harder.

Just write your book as a .doc and then upload through https://kdp.amazon.com/ - you will need to add a cover image and maybe a table of contents but other than that Amazon's software converts your .doc nicely into the Kindle format.

I've done this for a few horrible little books (I should stick to writing code) so let me know if you need specific help.

Not sure about hard copy.
