For my deployments i have one gateway that handles all PSTN connectivity, on it i have the analog trunks on FXO ports ISDN on E1 interfaces and this gateway then connected to the IP PBX using h.323 or mgcp. I use a Cisco router with atleats ipvoice image and the relevant interfaces.
