1. Yes, mod_rewrite is enabled in Apache.

2. I am using the following rewrite block:

<IfModule mod_rewrite.c>

        RewriteEngine On

        RewriteBase /

        RewriteRule ^index\.php$ - [L]

        RewriteCond %{REQUEST_FILENAME} !-f

        RewriteCond %{REQUEST_FILENAME} !-d

        RewriteRule . /index.php [L]

</IfModule>

Now, in WP, I have posts, which are placed under Categories. When you click on an item, you get it dispalced as http://site.name/?p=68 and this is what I want to change. I need http://site.name/sample-post/ instead of http://site.name/?p=123
