I was a bit worried at first...am actually at the limesurvey now...will halla if i hit a rock. good to know i wont be shot down lol! but seriously, its good to know there are guys out there that can get you when you are stuck in a rut ( of the technical kind)

Having gotten that out of my system: LimeSurvey http://www.limesurvey.org/ seems to fit your bill. In particular, it supports conditional questions (where the questions asked depend on previous answers). It is open-source, and written in PHP, so you can hack it if you need to. There are a gazillion other similar applications, many of them opensource, but LimeSurvey is what I have most experience with.

Google Docs/Spreadsheets also has a similar tool, though with fewer features than LimeSurvey: http://googledocs.blogspot.com/2008/02/stop-sharing-spreadsheets-start.html The advantage is that you don't need to install anything.
