Most IP phones have a provision where the network connection enters the phone, which then has another port where you can plugin a PC. Look at it like a small switching unit within the phone - though not really. No special cabling is needed. I do hope you know that in CAT5 it is possible to use only 2 pairs for data, right? You do not need to use all the 4 pairs. So depending on where you patch panel "patching", you can end up carrying one data line to PC and two voice lines - if your phones are digital (not IP) and use just a single pair for signalling.
