I think this points to a bigger problem in Network design and planning.

 

The issues most ISPs face are a factor of cost and poor engineering.

 

Poor engineering can be addressed by training which at least JG and others at EANOG are trying to address ;) When it comes to complex metro and nationwide networks, a mixture of factors can be quoted. Skills needed for such implementations are rare and reliance on the equipment vendors is common and transfer on knowledge is usually sketchy.  

 

While all have diverse and complex plans, getting the money needed proves to be quite a challenge. This is why when they are small they can manage their networks and offer relatively good service but once they require to scale to accomodate the increasing number of users, shortcuts become the order of the day.
