A SIP Trunk line from MTN is the best option, you could also take their complete hosted solution offer (Centrex),
 
However if you main requirement is the trunk lines you dont have to take Centrex(hosted PBX), you can have your own PBX at HQ and probably VOIP gateways on each brach with your own IP Phones at each site. In this case, you pay MTN for trunk lines and own the rest of your telephony infrastructure.

@ Brian, Thanks. An E1 would be an overkill for my scenario. I require one
or two pilot lines per branch and say 20-40 extensions.

@Wash, The basic requirements are;

1. Setup telephony in the various branches mostly in the major towns of KE
2. Have one/two pilot lines (not Telkom CDMA or GSM) which can be called
from anywhere.

Seems my major challenge is the pilot lines since i would not want to take
up E1 lines for my small requirement.  Your suggestions are quite
insightful let me look at the options.

Something like Asterisk would do it, if he need a pilot line, with E1,
it will have to be a box in his office as there are physical cards
that will be needed to accomodate E1.  That rule out VPS.  Wonder if
there is hosted  PBX providers with presence in Kenya?  I am not
certain how the E1 would be handled without local office.   He can set
a ssh tunnel and run a SIP truck between PBX across the offices.  If
only one PBX is to ne used, a VPN for the SIP clients would be
necessary

Wonder what would be cheaper, a single trunk between office and get
the SIP client to connect to the local office PBX.  That way, he do
not need to invest on VPN if he do not not have one already
