You can find scripts on the net to handle file uploads, move and even rename those uploaded files. It is also quite easy to save the path and name of the uploaded files in MySQL. Infact, you could also save the entire uploaded file in a database.

Once the data is in a database, its pretty much easy to link to it and create the links.
