This mostly happens when making calls, try and check if you have some background services running, mostly web based say you have set up automatic synchronization of contacts, automatic updates etc, then you are more likely to encounter this (open for correction) but i realized this after a while.
Mostly happens if you interchange country codes. For those of us who travel in the region alot and we save the phone numbers in the international format. Instead of Kenya's 254 you find that you put 256 for UG and so it will tell you that instead of telling you that the number is unavaileable or is wrong. But it might go through.
